Three reasons why case management has become so popular are; Inadequate funding for a variety of services, Increase in poverty level due to the changes in the economy
and also lack of likely hood that programs work together to facilitate and assist those in need of guidance.
2. Give a referral source for each one of the target population listed below:
Rural Elderly- Access to health care
Homeless- Referral source for lodging during dangerous weather conditions
Low Income- Resources to programs that provide food
Disabled- Resources that provide equipment for the client that encourages independence
3.Describe all of the five steps involved in the plan of care.
The steps involved in the plan of care are;
1. The goals agreed to by both and based on the information of the assessment,
2. Time limits on the overall case as well as on the individual goals,
3. The extended actions to be taken by all parties to achieve the goals,
4. Who accepts responsibility for each task, and
5. The consequences for a failure to comply.

Three questions that should be asked while monitoring the clients are;
1. Has the clients situation changed?
2. Are the referrals still appropriate?
3. Have the desired outcomes been achieved?

Ballew and Minks five escalating levels of advocacy include;
1.Make a direct appeal for a denied resource.
Interceding on the behalf of a clients when another agency is involved. Being involved and assisting the client can help avoid any miscommunication or confusion
between the provider/agency and the client. This assures that the clients request has been made clear and the proper treatment can be set up, policies and procedures
are explained to the client to avoid any confusion about what is expected during treatment and helps create a smooth transition for the client which will allow the
client to a positive start.
2.Appeal to higher authority. Taking the necessary steps to my own supervisor to help direct me in solving a matter would be a wise step to take. It is a likely hood
that both my supervisor and the other supervisor have spoken about other matters prior to mine and have a build a working relationship that can make solving matters a
smoother process for one another.
3.Use a grievance process. A case manager should be aware of the processes to file an administrative hearing and or grievance for their client.
4. Appeal to outside authority. Federal, state, or local agencies fund many of the providers in the health and social fields. A written appeal may be submitted and
need intervention if the client gets denied in spite of being covered by its regulations.
5.Legal action. A case worker should be aware of the steps to follow a civil suit and other legal actions when needed. If the case becomes overwhelming where it
interferes with your professionalism, it has been advised to seek legal counsel to aid the client.
