This assignment will include title page, four pages in length, use of APA, and three scholarly references.  The
tracking form is to be included as an appendix.
Select a computerized tracking form from the readings, the literature, the CDC EPI 7.4.1 Database, or you
precepted site. You will be populating the form with HAI items you wish to track.
Decide on the database you will use.  You may use the sample database in EPI 7.4.1, the Hospital Compare Data
from the course resources or from your precepted site with permission.
Select a HAI of interest to your precepted site.
Begin filling out the tracking sheet with data from the database of your choice.
Write the paper according to the Assignment 1 Rubric provided below. This assignment must be in APA format
with title page, approximately four pages in length not including the title page and references, and a minimum of
three scholarly references.
