Given:

The 65+ population is the fastest growing age group in the United States. At some point in time, every nurse will be challenged to address aging issues, with clients, family, friends, or self. In order to maintain or improve quality of life and health, and to manage health care resources, nurses must assess factors placing older adults at risks and develop approaches to minimize the risks.

This performance task focuses on identifying the risks older adults face in maintaining independent living, applying clinical reasoning and decision making processes using the health history to analyze information, and proposing nursing actions to minimize the risks to maintain or maximize the opportunity for independent living for the older adult.

The student is to collect health history related to risk factors impacting an older adults ability to engage in self-care activities and independent living. The older adult must be at least 70 years of age. Client confidentiality must be maintained at all times.

Task: Write an essay (suggested length of 4-5 pages) in which you address the following:

A. Begin the paper with an introductory paragraph that briefly describes at least four major topic areas you are covering.

B. Use previous knowledge and skills to complete a focused assessment related to capacity of an older adult to assume responsibility for self-care.

Complete a focused health history on an older adult client related to their ability to engage in self-care activities.

Explain actual or potential risks for engaging in self-care activities by an older adult based on information gathered during the focused health history.

Specify which systems to include in a focused physical assessment based on the focused health history data.

Analyze health history data to determine risks to the older adult in caring for self.

C. Apply clinical reasoning and decision-making processes to evaluate clinical evidence and make decisions.

D. Propose relevant and appropriate nursing actions to meet client needs based on health history and physical assessment evidence.

Explain factors affecting the determination of nursing actions.

Relate the five stages of the nursing process to the selection of nursing actions.
