AT 1540 you are called to a rural property at Bella Creek, which is approximately 66 kilometres north-west of Nambour. The case is given as a 43 year old male injured when the quad bike he was riding overturned on a steep slope. He was found underneath the bike by another member of his family, who lifted the bike off the patient and called 000.
On arrival the patient is conscious and complaining of severe (9/10) pain to his pelvis. The patient denies any loss of consciousness or pain in other body regions. There is bruising over the pelvis, but no penetrating wounds or external blood loss. The patient has facial abrasions, but has no signs of traumatic brain injury. The patient has no relevant medical history. There is no backup available for this case due to the current caseload in the region. The helicopter is already tasked to another case. A local SES member is available to assist.
During your assessment of the patient you are concerned about deterioration in the patient’s condition, and need to make a decision to replace intravascular volume.
Part A. Describe methods for assessing perfusion status that are reliable and practical for use by paramedics. Choose at least three methods for assessing perfusion status with the aim of deciding when to replace intravascular volume. This section requires a brief introduction to the physiological process of perfusion homeostasis, and a critique of at least three methods for determining inadequate perfusion using current evidence from peer-reviewed journals. This requires a summary of the strengths and limitations of the methods you selected that can be shown in a table format. (1000 words).
Part B. You are working for an ambulance service that allows the use of crystalloid in the form of either 0.9% NaCl or Hartmann’s Solution. Your guideline also allows the use of blood. You have all three types of product on your ambulance, and as such you need to select the type of product you are going to administer to the patient. This section requires a description of the properties of the three types of product, and a critique of the literature using current evidence from peer-reviewed journals. You must demonstrate an understanding of the properties of each product, including an understanding of the types of blood product that may be best suited to this case. Assume that you have these products on your ambulance. This section requires a summary of the strengths and limitations of the products when used to replace intravascular volume in cases of blunt trauma, and justification for the approach to intravascular volume replacement in this case (1000 words).
