Stakeholder Involvement in Developing Mission Statement, Goals, and Objectives -5 pages
• Explain why it is important for representatives of your target population to be involved in developing the goals and objectives for the program. • Identify which stakeholders you would involve in the planning process, and discuss two or more strategies for facilitating their involvement.
Program Design • Create a visual representation of your program design (e.g., a table or graph) that includes the program’s mission statement, goal(s), objectives, and activities to meet the objectives. • Develop a Gantt chart with time lines for implementing all activities that will assist in meeting each of your program objectives.
Application 3: Program Budget and Financial Analysis-4 pages Budget Develop a simple revenue and expense budget for 6 months from start-up and develop a break-even analysis. Articulate a justification for the revenue and expenses indicating where the funds are coming from to support the project and what resources the expenses are related to. • Explain any variance from the budget that is not budget neutral.
Financial Analysis • Articulate whether the monies put into the program will result in positive health outcomes for the population (resulting from your application of the use of cost-effectiveness, cost-utility, or cost-benefit to the program design process).

Evaluation Methods (developed in Week 9) Identify an evaluation theory or model that is aligned to your program goal(s) and objectives. Exhibit a performance measurement, monitoring, and evaluation time line that: o Demonstrates the appropriate use of performance measurement, monitoring, and summative evaluation o Distinguishes between the long-term effects of impact evaluation versus short and intermediate health outcomes as a result of the implementation of the program
Evaluation Plan • Develop an evaluation plan that includes goals, objectives, and activities. Specify the type of data needed. • Add your evaluation plan to the visual representation (e.g., table or graph) of your program plan design. • Add time line information for the evaluation plan to your Gantt chart.
